The rules were set out in RFC 4645, Section 2.

In short, the code elements for macro-geographical regions are in; those
for individual countries (because they already have two-letter code
elements from ISO 3166-1) and for non-geographical categories like
"economic groupings" are out.

You can get an up-to-date list by looking in the Language Subtag
Registry [1]. The three-digit region subtags are listed immediately
after the two-letter ones.
